Immunization and Health Requirements

All students should begin collecting health documentation and scheduling medical appointments immediately. It may take up to two months or longer to fulfill the immunization and titer requirements for both Student Health and Counseling and Clinical Education via Castle Branch.

Student Health and Counseling has New Student Immunization Requirements that must be met before a student can enroll in courses. These requirements must be submitted by May 12. If you are not able to meet the deadline,  you must contact Student Health and Counseling as soon as possible to temporarily lift your registration hold in order to enroll in classes and/or request for a deadline extension.

Castle Branch (CB) is a service that UCSF uses to gather and store necessary documents for Clinical Education. These are standardized requirements that will need to be completed before you will be able to work with patients. Depending on where and when your clinical affiliation is, you may need to complete additional requirements. Documents must be uploaded to Castle Branch by June 1. Instructions for creating a Castle Branch account and uploading documents will be provided during Accepted Students Day in March.

Note: Some clinical education immunization requirements overlap. In these instances, please follow the strictest requirement (i.e. TB Screening).

For TB Screening, all Physical Therapy students must complete a 2-Step TB test within 1-3 weeks for Clinical Education regardless of the alternatives listed for fulfilling the TB screening listed by Student Health & Counseling.

We ask that you get the 2-step TB test  within 1-3 weeks rather than Quantiferon as several clinics specifically require the 2-step test.

Additionally, if you have ever received a positive result, you must submit a clear chest x-ray.

Student Health & Counseling requires TB screens to be completed within 3 months prior to beginning the program in June.

 

New Student Immunization Requirements

You will receive an email from Student Health and Counseling Services outlining the pre-entrance health requirements and instructions on how to submit your data. The requirements are to be satisfied prior to starting classes. For general information on the health requirements visit new student immunization requirements. Please note that form for the 2017-18 academic will be updated slightly and available by Accepted Students Day. 

 

 

Clinical Education

Instructions for establishing a Castle Branch account will be distributed during Accepted Students Day in March.·Documents must be uploaded to Certified Profile by June 1.

 

  1. Measles, Mumps & Rubella (MMR): Upload documentation of BOTH of the following:
  • 2 vaccinations AND
  • Positive antibody titers for all 3 components (lab reports required)

*If any MMR titer is negative or equivocal, you must receive one booster and a repeat titer. If the titer is still negative or equivocal, you must receive a second booster and titer. If the titer is still negative, please inform us promptly. Vaccine doses must be at least 28 days apart.

 

2. Varicella (Chicken Pox): Upload a positive antibody titer (lab report required). Only positive titers are accepted.

3. Tetanus, Diphtheria & Pertussis (Tdap): Upload copy of a Tdap booster within the past 10 years.

4. Hepatitis B: There must be documentation of BOTH of the following:

  • 3 vaccinations AND
  • Positive antibody titer (lab report required) 

*If the series is in process, new alerts will be created for you to complete the series. Only positive titers are acceptable.

 

5. TB Skin Test: Upload documentation of one of the following:

  • 2-step TB Skin test (1-3 weeks apart) OR
  • Past 2-step TB Skin Test PLUS all subsequent annuals

6. UCSF Student Health Form: Submit the 4-page UCSF Student Health Form from UCSF My Health Record.

7. Physical Examination: This can be either a completed physical examination form or simply a letter on office letterhead signed by your doctor stating that you are in good health.

8.CPR Certification: Upload a copy of a non-expired CPR certification card from the American Heart Association Basic Life Support for Healthcare Providers (BLS)  Classroom Training course. Copy must be front and back of the card, and signed.

9. Proof of Health Insurance: Submit a copy of your current health insurance card or proof of coverage. *Due July 1

10. Influenza: Upload either documentation of a flu shot administered during the current flu season OR a declination waiver form. *Due November 1
