New Student Transcript Requirements

All incoming students are required to submit all official transcripts to San Francisco State University and updated official transcripts to UCSF.  Please see the requirements for both universities below.

  • San Francisco State University - You must send official transcripts for EVERY US college and university you attended, including transcripts noting your degree conferral and final grades for all prerequisite coursework.
    • SFSU accepts electronic transcripts from regionally accredited U.S. colleges and universities sent through third-party delivery services such as eSCRIP-SAFE, Docufied, National Clearinghouse and Parchment. They also accept electronic transcripts sent directly from the university. All electronic transcripts must be sent directly to: [email protected] from the institution to be considered official (Any electronic transcript sent from a student’s email address will be considered unofficial).
    • If you are currently enrolled in courses, you must submit additional transcripts at the end of the term with final grades and/or degree conferrals posted.

    • Once you have submitted your transcripts to SFSU, you will receive an email with instructions for completing the Cal State Apply application and admissions process at SFSU.
  • UCSF - You must submit one official, sealed transcript from each institution that you have attended only if changes have been made (updated grades, degree conferral) from the official transcripts that are in your PTCAS application. Final grades and/or degrees conferrals must be posted on the transcripts. If you are currently enrolled in courses, you can submit the transcript at the end of the term with final grades and/or degree conferrals posted.
    • Electronic (Preferred): You can request official transcripts sent electronically to [email protected]. They must come from an official source (directly from the university or third-party delivery service). Any electronic transcript sent from a student’s email address will be considered unofficial.

The United States Postal Service (USPS) and UPS require all UCSF departments in San Francisco use the zip code 94143 regardless of location.

Packages from other commercial carriers (e.g., FedEx) and accountable mail (i.e., registered, certified, insured and/or express mail for which a signature is required) should be addressed with the geographic zip code 94158.
